    It is not intended to provide the total ventilatory requirements of the patient. WARNING: We do not recommend you use the Respironics V60 Ventilator on patients who require ventilation at predetermined tidal volumes. The ventilator provides continuous positive airway pressure (CPAP) and positive pressure ventilation (S/T, PCV, and AVAPS, and PPV) and is indicated for assisted ventilation only.

    Federal law (USA) restricts this device to sale by or on the order of a physician. CAUTION: The Respironics V60 Ventilator is designed to operate in the temperature range of 5 to 40 ºC (41 to 104 ºF). To minimize the risk of overheating the device, do not operate adjacent to heaters or other heat sources.

    To reduce the risk of fire, do not use a high-pressure oxygen hose that is worn or contaminated with combustible materials like grease or oil. WARNING: The Respironics V60 Ventilator is designed to use ambient air and high pressure 100% oxygen. No other gases should be used. WARNING: Do not use the ventilator with helium or mixtures with helium.

    Any additional accessories in the patient circuit may substantially increase flow resistance and impair ventilation.     Chapter 3. General information Intended use The Respironics V60 Ventilator is an assist ventilator and is intended to augment patient breathing. It is intended for spontaneously breathing individuals who require mechanical ventilation: patients with respiratory failure, chronic respiratory insufficiency, or obstructive sleep apnea in a hospital or other institutional settings under the direction of a physician.

    General description The Respironics V60 Ventilator (Figure 3-1) is a microprocessor-controlled, bilevel positive airway pressure (BiPAP) ventilatory assist system that provides noninvasive positive pressure ventilation (NPPV) and invasive ventilatory support for spontaneously breathing adult and pediatric patients.

Алгоритм использования спейсера для ингаляций

Базисная терапия бронхиальной астмы основана на применении препаратов, которые  помогают снимать воспаление дыхательных путей и облегчать дыхание. Также для терапии «скорой помощи» могут быть назначены  препараты, которые помогают расширять  дыхательные пути. Чтобы усилить эффект, эти лекарственные средства  используют ингаляционно, в том числе с помощью дозированного аэрозольного ингалятора (ДАИ). Эффективность ингаляции с помощью ДАИ зависит от того, может ли пациент четко координировать активацию дозирующего устройства со вдохом. Только при выполнении этого условия лекарственный препарат попадает в дыхательные пути в необходимой лечебной дозе.
 

Далеко не все больные бронхиальной астмой могут правильно использовать ДАИ. При неверном применении устройства лекарство распыляется со слишком большой скоростью и не координируется с вдохом, из-за чего часть препарата попадает в желудок или остается на задней стенке глотки. Это приводит к снижению эффективности лечения и ухудшению контроля над астмой.

Чтобы ингаляция была проведена правильно, и препарат поступал в зону назначения – дыхательные пути – используются дополнительные устройства, в частности, спейсер. Что собой представляет спейсер, как им пользоваться и какие приспособления выбирать?

Спейсер и его преимущества


Спейсер –  приспособление, позволяющее повысить качество ингаляционной терапии бронхиальной астмы. Оно состоит из камеры с клапаном, которая с помощью переходника герметично соединяется с ДАИ. При распылении препарат попадает в камеру спейсера, и только во время вдоха открывается клапан, выпуская аэрозоль. Таким образом снижается скорость распыления лекарственного вещества, что дает возможность оптимально координировать его со вдохом и оптимизировать ингаляцию.
 

Использование ингалятора со спейсером имеет несколько преимуществ по сравнению с применением только ДАИ [1]:

Повышение эффективности ингаляционной терапии


Благодаря спейсеру практически вся доза лекарственного препарата проникает в дыхательные пути, в то время как при использовании ДАИ без спейсера часть лекарства остается во рту, на стенках глотки или попадает в пищеварительный тракт.

Снижение риска развития побочных эффектов


Поскольку спейсер обеспечивает поступление практически всей дозы лекарственного препарата в дыхательные пути, лекарство практически не попадает в желудок и не оказывает системное действие. Это позволяет избежать ряда неблагоприятных реакций, связанных с проникновением препарата в кровоток.

Кроме того, благодаря применению спейсера снижается вероятность развития кандидоза слизистой оболочки полости рта – распространенного побочного эффекта, связанного с использованием ДАИ с глюкокортикостероидами.

Как применять спейсер: 11 шагов к свободному дыханию


Чтобы правильно делать ингаляцию с помощью спейсера, нужно следовать простой и эффективной инструкции [2].
 

  1. Снимите крышку ингалятора. Заправьте ингалятор так, как указано в инструкции по применению ДАИ.
  2. Снимите крышку со спейсера и осмотрите мундштук, чтобы убедиться в том, что в нем ничего нет. Соедините ингалятор со спейсером при помощи адаптера.
  3. Проводите ингаляцию стоя или сидя прямо. Займите одно из этих двух положений.
  4. Сделайте глубокий вдох, слегка наклоните голову назад и полностью выдохните – так вы очистите легкие.
  5. Поместите мундштук спейсера в рот и сомкните губы так, чтобы они плотно прилегали к мундштуку.
  6. Вдыхая, надавите на верхнюю часть баллона с лекарственным препаратом, чтобы распылить лекарство. Продолжайте дышать как можно медленнее и глубже 3–5 секунд.
  7. Задержите дыхание и досчитайте до 10.
  8. Выньте спейсер изо рта. Выдыхайте медленно.
  9. Если врач рекомендовал принимать больше 1 дозы лекарственного препарата, через 1 минуту повторите ингаляцию (шаги 3–8).
  10. Разберите спейсер и ингалятор. Наденьте крышку на свой ингалятор и установите крышку на спейсер. Следуйте инструкциям производителя ДАИ об уходе за ингалятором.
  11. Если вы принимаете ингаляционный кортикостероид, прополощите рот водой, чтобы предупредить развитие инфекции в ротовой полости.

Если вы принимаете ингаляционное средство «скорой помощи», подождите около минуты, прежде чем делать следующий вдох.

Спейсер: как пользоваться ребенку?


При лечении бронхиальной астмы у детей особенно важно обеспечить оптимальную эффективность ингаляции, ведь малыши часто с трудом координируют вдох с нажатием на баллончик ДАИ. Поэтому спейсеры в детской практике особенно актуальны. Их можно использовать для ингаляции у детей любого возраста, в том числе грудных младенцев.
 

Чтобы обеспечить оптимальную эффективность ингаляционного лечения, детям раннего возраста (от 0 до 3 лет) рекомендуется использовать маску для лица, поскольку малышам бывает трудно пользоваться мундштуком. Важное условие эффективности ингаляции — точное соответствие размера маски. Во избежание потерь лекарственного препарата маска должна плотно прилегать к лицу. В таком случае в дыхательные пути попадает вся доза лекарственного препарата, и результат лечения оптимален.
 

Детям старше 3 лет (как и взрослым) вместо маски можно использовать мундштук. Задача родителей – научить малыша корректно использовать мундштук, плотно обхватывая его губами, и правильно дышать.
